questions, trepidation: going back to the neuro

Last week I scheduled an appointment with my neurologist. In one week I'll be back there trying to determine what the heck to do about my situation. I know Dr. W. and I will need to work together on those decisions, but I feel as though I need to have some idea what I am and am not willing to explore when I walk into the appointment. And I have a lot of questions.

(1) If my neck pain / issues are the root of most of my migraines, does it make sense to keep trying preventative medications? (Especially since I have tried so many and had no success.)

What about the fact that migraine is a neurological disorder? Does that mean the neck issues by definition cannot be the main cause?

(2) How can we get the ball rolling with referrals to a pain specialist and rheumatologist? I've got to get some support for my pain issues.

(3) Should I stick with Lamictal and take a higher dose or move on to a different preventative? If we move on, what the heck can we try next?

(4) Can I try a TENS unit for my neck pain? Any other ideas that I haven't already tried (injections, PT, chiro, massage, acupuncture, muscle relaxers) for managing the neck pain?
